Opera has a small-screen mode (can't remember if that's what it's
called), which I believe is a fairly accurate approximation.

Just follow the normal accessibility guidelines.

Why not? As long as they have appropriate alternative text (which they
should anyway), it won't be an issue, regardless of whether the UA
supports images or not.

No problems, providing the tables linearise logically. Though whether a
form is tabular data is still a matter of debate.
